CASE MANAGER

The Case Manager is responsible for coordination, monitoring, and implementation of all aspects of individual consumer plans of service. Will perform assessments, team meetings, and makes referrals for other services. Must meet MDCH requirements for QMRP/QMHP, and is a psychologist, physician, educator with a degree in education from an accredited program, social worker, physical therapist, occupational therapist, speech pathologist, audiologist, registered nurse, therapeutic recreation specialist, licensed or limited-licensed professional counselor, licensed physicians assistant; or an individual with a human services degree hired and performing in the role of a QMHP/QMRP prior to January 1, 2008. A minimum of one year of work related experience is required. Licensure appropriate to professional degree and a valid Michigan Drivers license is required. $39,124-$52,811.

PEER Support Specialist

Will provide a variety of services following the person-centered-plan process, co-facilitating, and assisting individuals towards independence, conduct home visits with or without personnel to monitor consumer recovery. Qualified candidates must be currently or previously served by the public mental health system and through a recovery process. HSD or GED Equivalent. $11.74-$15.26 Hourly.

IS SEEKING QUALIFIED CANDIDATES FOR THE FOLLOWING POSITIONS:

EMPLOYMENT QUALIFICATIONS: Individual with specialized training or one year experience in treating or working with a person who has mental illness; and is a psychologist, physician, educator with a degree in education from an accredited program, social worker, physical therapist, occupational therapist, speech pathologist, audiologist, registered nurse, therapeutic recreation specialist, licensed or limited-licensed professional counselor, or a licensed physicians assistant. An individual with a bachelors degree in a human services field who was hired prior to January 1, 2008 and performed in the role of a QMHP prior to January 1, 2008 would also qualify. Must be IMH Level II or III Endorsement eligible. QMHP/QMRP and a minimum of one year work related experience in the human service field required. Training/experience with SED children and/or Infant Mental Health, preferred.
